Movement Participant Agreement

A PARTICIPANT AGREEMENT IS REQUIRED FOR ANYONE ENTERING THE GYM

FOR YOUTH PARTICIPANTS (AGES 0-17)

• Participants under 18 must have a facility agreement completed by their parent or court appointed legal guardian*
• Adult's name and birthday will be entered first
• Youth's information will be entered later

*Legal guardians must present proof of guardianship to sign a child’s participant agreement.

If the parent or guardian will not be present, please bring a photocopy of the adult’s government-issued photo ID for verification.
